Introduction: National Electrical Manufactures Association (NEMA) performance measurements are normally performed using 18F radioisotope. While 18F is more commonly used in clinical scan, 68Ga is growing as a radioisotope for certain clinical applications in recent times. 68Ga radioisotope has physical properties such as positron range, higher positron energy, half-life, and prompt gamma coincidence different from 18F. In this work, we evaluate the effect of 68Ga radiotracer on the performance of Siemens Biograph Horizon PET-CT system

Methods: In this study, the performance characteristics of sensitivity and image quality were evaluated by strict adherence to the NEMA NU 2-2018 standards other than using 68Ga radioisotope. Data was acquired on Siemens Biograph Horizon PET-CT scanner (16.4 cm axial field of view). Sensitivity was measured using a polyethylene tube placed inside the standard five aluminum sleeves. The standard NEMA image quality phantom was used to acquire data and reconstruct using iterative method with point spread function modelling. Contrast recovery, background variability and average lung residual error were calculated from the reconstructed data. Activity concentration used for the image quality and sensitivity calculation were according to prescribed NU-2-2018 standard for the 18F. The results obtained in this study with Ga68 were compared with the standard performance characteristics using 18F for the same scanner.

Results: Overall, results were similar to results published with F18, with some differences: sensitivity obtained using 68Ga was 5% less than 18F; for NEMA IQ, contrast recovery values with 68Ga were lower for all hot spheres; lung error was 2% higher for 68Ga compared to 18F. Measurement of spatial resolution will also be reported

Conclusions: From the initial analysis it was seen that the performance of 68Ga radioisotope is only marginally different compared to the 18F. Only one experiment with 68Ga was used in this study: we plan to add more data points and provide more insight and robustness to our assessment.
