Objectives: To evaluate the clinical value of 99mTc-MIBI SPECT/CT dual phase imaging in different hyperphasic time and Corresponding time point fusion imaging in diagnosis hyperparathyroidism.

Methods: Sixty-eight patients with hyperparathyroidism were included. 99mTc-MIBI at different time points (20min and 60min), (20min and 90min) dual phase imaging and the corresponding time with SPECT/CT fusion imaging was performed before operations in all patients. The imaging findings were compared to the pathological results which as the golden standards.

Results: 68 patients were diagnosed as PHPT (primary hyperparathyroidism) in 23 cases and 45 cases of SHPT (secondary hyperparathyroidism). The diagnostic sensitivity and accuracy of the dual phase imaging group (20min and 60min) were 66.10% and 71.14% respectively, and the corresponding time points of 60min SPECT/CT fusion imaging of the same machine were 80.45% and 82.38% respectively; the diagnostic sensitivity and accuracy of the dual phase imaging group (20min and 90min) were 68.25% and 74.68% respectively, and the corresponding time points of 90min SPECT/CT fusion imaging of the same machine were 83.58% and 85.70% respectively. There was no significant difference in sensitivity and accuracy between the two groups of dual phase imaging time detection methods. (P > 0.05). There was significant difference in the sensitivity and accuracy of the same time point fusion imaging and the corresponding time point dual phase imaging (P < 0.05).

Conclusions: The sensitivity and accuracy of the 99mTc-MIBI double-phase (20min and 60min) group and the (20min and 90min) group were no obvious difference. This study showed that the time of the delay phase of the 99mTc-MIBI dual phase was early to 60min, which did not reduce the diagnostic efficiency. 99mTc-MIBI SPECT/CT fusion imaging is more effective than 99mTc-MIBI dual phase imaging in the diagnosis of hyperparathyroidism.[Key words]99mTc-MIBI; hyperparathyroidism; SPECT/CT
