Objectives: The purpose of the study was to evaluate changes of CBF with Arterial spin labeling (ASL) and SUVr with 18F-FDG PET for patients with ischemic cerebrovascular disease after surgery using hybrid PET/MR.

Methods: 10 patients with unilateral internal carotid artery (ICA) occlusion were scanned by 18F-FDG PET/MR (GE Healthcare, WI, USA) examination before and after surgery. Data of 18F-FDG and ASL sequence was reconstructed by the GE Healthcare advantage workstation (version:4.6). An asymmetry index (AI) used to detect left-right asymmetries in the FDG PET SUVr and the ASL MRI CBF. S A two-sample t-test was used to compare the CBF and SUVr value between the preoperative and postoperative. Spearman’s rank correlation was also used to compare CBF and glucose metabolism in the same region.

Results: An example of the preoperative PET/MR map of a STA-MCA bypass patient is shown in Fig. 1. The analysis primarily revealed a statistically significant increase in the CBF and SUVr on the affected side after bypass surgery (P<0.05) (Fig. 2). A significant correlation was found between the abnormalities AI pre-operative CBF and pre-operative SUVr on the ipsilateral hemisphere (P < 0.05) (Fig. 3).

Conclusions: Combined ASL sequence and 18F-FDG PET imaging hold promise as an indicator of the effectiveness of bypass surgery in cerebral ischemia.
