Objectives: To explore the relationship between age, total prostate-specific antigen (tPSA), free prostate-specific antigen (fPSA), Gleason scores and systemic bone metastasis of prostate cancer (PCa), the diagnostic value of MRI and bone scan in the pelvic area bone metastasis was compared.
Methods: A retrospective analysis of 51 patients with PCa diagnosed in the First Hospital of Shanxi Medical University from January 2018 to January 2020, aged 72.29±1.04 years. All patients underwent radionuclide whole-body bone scan and pelvic MRI examination within half a month (Fugre 1). The clinical criteria of metastatic lesions is used to diagnose bone metastasis, analyze the related factors of whole-body bone metastasis, and compare the value of MRI and radionuclide bone scan to diagnose bone metastasis in the pelvic area.
Results: (1) 51 patients with PCa were divided into bone metastasis group(24 cases) and non-bone metastasis group(27 cases). The tPSA, fPSA, and Gleason scores of the bone metastasis group were significantly higher than that of the non-bone metastasis group (P<0.05,Table 1). Among them, when the values of tPSA and fPSA were 68.59ng/ml and 6.42ng/ml, the sensitivity and specificity of diagnosing bone metastasis were 75.00%, 81.50% and 83.30%, 81.50%, respectively (Table 2,Fugre 2). (2) According to pelvic area analysis, patients were divided into bone metastasis group(22 cases) and non-bone metastasis group(29 cases). The sensitivity, positive predictive value and diagnostic efficiency of MRI and bone scintigraphy in diagnosis of pelvic area bone metastasis were 86.36%, 90.48% ,90.20% (P<0.05) and 90.91%,83.33%,88.24% (P<0.05),respectively(Table 3).
Conclusions: (1) Elevated levels of tPSA, fPSA, and Gleason scores are a good indicator for predicting bone metastasis, especially when the cut-off value of tPSA and fPSA is 68.59ng/ml and 6.42ng/ml, the diagnosis was the best. (2) For the study of the pelvic area, it is found that MRI and bone scintigraphy can complement each other in clinical practice to achieve early diagnosis of PCa bone metastasis.
