Abstract

Objectives Phase analysis of gated myocardial perfusion SPECT (GMPS) has been established for measuring left-ventricular (LV) dyssynchrony. Assessment of LV dyssynchrony and function before and after cardiac resynchronization therapy (CRT) can provide valuable clinical information. This study is aimed to evaluate repeatability of LV dyssynchrony and function parameters on serial GMPS.

Methods 23 patients, who met standard criteria for CRT, were prospectively enrolled. One hour after resting injection of Tc-99m sestamibi, a standard GMPS scan was performed on a GE Infinia system, and repeated 30 minutes later after repositioning the patient. Ordered-subsets expectation maximization and Butterworth filtering were used to process all studies. The Emory Cardiac Toolbox with phase analysis was used to calculate phase standard deviation (PSD), phase histogram bandwidth (PHB), ejection fraction (EF), end-systolic volume (ESV), and end-diastolic volume (EDV). Image processing was done side-by-side for two serial scans by an experienced operator.

Results The differences in the measured parameters between the two serial studies are listed in the following table. Paired t test showed no significant difference in any parameter between serial studies. The measured parameters were highly correlated between the two serial studies.

Conclusions The results indicated high repeatability of LV dyssynchrony and function measurement. The measured variability of these parameters can be used to assess changes in LV dyssynchrony and function after CRT.
