Eine neue Methode zur Erzeugung von Thromben durch gezielte Überdehnung der Gefäßwand

Zusammenfassung

Es wird eine neue Methode zur Erzeugung von Thromben beschrieben. Sie besteht in der durch Druck- und Zeitmessung definierten Überdehnung eines Gefäßsegmentes mit Hilfe eines von einem Seitenast her eingeführten Ballonkatheters, somit in einer konstant reproduzierbaren, lokal-destruktiven Einwirkung auf die Gefäßwand.

Gezielte Überdehnung der Vena jugularis des Kaninchens mit 1,2 kg/ cm2 während 2 min führte in 36 von 40 Fällen zur Bildung von meist partiellen Abscheidungsthromben verschiedener Größe. In der Aorta abdominalis des Kaninchens gelang es auch bei Anwendung großer Dehnungstraumen (1,3 kg/cm2 während 13 min) nur in vereinzelten Fällen längere Zeit an der Wand haften bleibende Thromben zu erzeugen.

Gezielte Überdehnung gleicher Intensität bewirkt gesetzmäßig die gleichen Veränderungen der Gefäßwand. Versuche mit Dehnungstraumen verschiedener Intensität ergaben, daß bei gleichbleibenden Strömungsverhältnissen des Blutes eine enge Beziehung zwischen Schweregrad eines Gefäßwandschadens und Ausmaß der Bildung von Abscheidungsthromben besteht.

Summary

A new method for the production of thrombi is reported which consists of overdilatation of a vessel-segment by the use of a ballooncatheter introduced through a lateral branch or tributary. The overdilatation is standardized by control of pressure and time, so that a consistent local destructive effect on the vessel-wall is produced.

In rabbits controlled over-dilatation of the jugular vein using 1,2 kg/ cm2 pressure for 2 minutes led in 36 out of 40 cases to formation of thrombi of varying sizes, most of which were incomplete. In the rabbit abdominal aorta, by contrast, even when excessive dilatation-trauma (1,3 kg/cm2 for 13 minutes) was employed, thrombi which were adherent to the vesselwall for any significant period were observed only in occasional instances.

Controlled over-dilatation of any given degree produces lesions of reproducible severity. Studies with varying degrees of dilatation-trauma showed that in the presence of a constant flow-rate the severity of vessel damage and the extent of thrombus formation are closely correlated.
