1: mild (unable to work or needing occasional analgesics) | 1: occasional (less than once per day) |
2: moderate (mobile at home or not needing opioids) | 2: intermittent (at least once per day) |
3: severe (in a chair most of the time or needing low-dose opioids) | 3: frequent (1–3 times per day) |
4: unendurable (in bed more than 80% of the time or needing high-dose opioids) | 4: constant |
Pain score = severity score × frequency score.
