Here, we hypothesized that 18F-FDG uptake is mutually exclusive from functional blood perfusion in solid malignancies of human beings. Methods: The study was approved by IRBs and written consents were obtained from all patients. A total of 19 patients with pathologically confirmed pretreated lung cancer were included in the reported. After 6 hours fasting, patients were positioned supine on the scanning bed and PET detectors were placed over the lesion of interest, an intravenous bolus injection of 18F-FDG (0.1mCi/Kg body weight) to was made; followed a 5 min regional scan to obtain 18F-FDG perfusion imaging. The second PET scan was performed 1 h after 18F-FDG injection to obtain whole body PET imaging for glucose metabolism. Results: The intratumoral 18F-FDG accumulation was generally heterogeneous in lung cancer. Regions with high 18F-FDG perfusion appearing in initial 5min PET scan generally associated with low 18F-FDG avid 1h late. Regions with high 18F-FDG accumulation at 1h revealed low perfusion in initial 5min PET imaging after 18F-FDG injection; lack of 18F-FDG perfusion radioactivity at initial 5 min PET imaging. Regions with low radioactivity perfusion at early imaging and minimal 18F-FDG accumulation were generally identified as necrosis by CT. Conclusions: In patients with lung cancer, intratumoral 18F-FDG accumulation and 18F-FDG “vascular-graphy” are mutually exclusive. The authors recommend a dual phase 18F-FDG PET imaging protocol (early perfusion imaging and 1h metabolism imaging) for better understanding underline tumor biology.
