Volumes of interest were placed on tumors to measure maximum SUV (SUVmax) and average SUV (SUVavg) of FDG. Texture analyses were applied to FDG voxel distribution in tumors to calculate the heterogeneity indices of skewness, kurtosis, and area under curve of cumulative SUV-volume histograms (AUC-CSH), that is, a parameter defined as % of total tumor volume above % threshold of SUVmax. A lower AUC-CSH was considered to correspond to a higher heterogeneity. All pts were categorized into LC and non-LC groups, and the parameters were compared between the two groups by Mann-Whitney’s U test. Results Of the 49 pts, 45 were treated with IMRT combined with chemotherapy, and the others with IMRT alone. The long diameter of tumor before therapy was 24.18±8.34 mm and 28.42±9.40 mm for LC (n=38) and non-LC (n=11) groups, respectively. Voxel distribution of FDG demonstrated skewness of 0.79±0.25 and 0.82±0.60 (ns), kurtosis of 2.85±0.66 and 3.40±0.83 (p<0.05), and AUC-CSH of 0.34±0.04 and 0.29±0.07 (p<0.05) for LC and non-groups, respectively. In contrast, no significant difference in SUVmax or SUVavg was observed between the two groups. Conclusions Pretherapeutic intratumoral heterogeneity of FDG on PET/CT simulation correlated with tumor response, and might predict local control.
