AB 1465 Objectives Previously we showed that 123I-IMP SPECT is a sensitive and accurate method for the detection of uveal malignant melanoma (MM). The feasibility and usefulness of 18F-FDOPA PET for detection of uveal MM have not yet been established. The purpose of this study is to compare the usefulness of 123I-IMP SPECT, 18F-FDG PET, and 18F-FDOPA PET for detection of uveal MM.Methods Nineteen patients with suspected uveal MM were examined by 123I-IMP SPECT, 18F-FDG-PET, and 18F-FDOPA PET.Results In 11of 19 study patients, 123I-IMP SPECT showed markedly increased uptake in the site corresponding to the ocular tumor. In 8 of the 11 patients showing increased uptake of 123I-IMP, the ocular tumors were confirmed histopathologically or clinically to be choroidal MM. The ocular tumors in the other 3 patients showing increased uptake of 123I-IMP was diagnosed as small choroidal melanocytic lesions. The 8 123I-IMP SPECT-negative cases comprised 1 small melanocytic lesions, 1 choroidal hemangioma, 1 adenoma, and 6 choroidal nevus. Seven of all the cases of choroidal MM showed positive uptakes in 18F-FDG PET and 18F-FDOPA PET. The uptakes of both 18F-FDG and 18F-FDOPA were much weaker than that of 123I-IMP. The range and mean of SUVmax for 18F-FDG and 18F-FDOPA in choroidal MM were 1.70-3.54 and 0.74-1.38, respectively. The difference between the uptakes of both tracers was statistically significant. Thus the uptake of 18F-FDOPA was weaker than that of 18F-FDG.Conclusions 123I-IMP SPECT is superior to 18F-FDG-PET and 18F-FDOPA for the detection of choridal MM. 18F-FDOPA PET is not useful for the detection of choroidal MM.
